At Capodice, Efaw, Ocheltree & Elger, PLLC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.

Our office will submit a claim to your insurance as a courtesy, we are not in-network with all insurance but will bill an insurance regardless of in-network or out-of-network status ***. Below is a list of insurances we are in-network with. While we may be in-network with a specific insurance company, this does not guarantee that a patient’s particular plan will be paid as in-network. It is always best to contact your insurance company directly to verify if our doctors are in-network with your particular plan.

• Cigna Dental
• Delta Dental (in-network March 1st 2024)
• Aetna Dental
  • PPO only
• Medicare (It is important to note that Medicare does not cover any teeth extractions)

*** We do not participate in any of the Medicaid plans and cannot submit a claim on your behalf.
